3. What should the grievance include?

You may file your grievance through our intake form or directly on iSight. Alternative methods of form submission can be made available on request. Whatever format you choose, your grievance must include the following information:

  1. Your name, address, email address and telephone number. If a representative is filing the grievance on your behalf, their name, address and telephone number must also be included.
  2. A description of the behavior(s) or action(s) or violation(s).
  3. The date(s), time(s) and location(s) of the incident(s).
  4. If the incident(s) involved University faculty member(s), student (s) or employee(s), their name(s) should be included, if possible.
  5. If there are any witnesses to the incidents, their name(s) should be included, if possible.
  6. If your grievance is being filed on behalf of another person or a group of people, all of the grievants should be described or identified by name, if possible.
  7. The remedy you desire.
  8. Your signature or the signature of your authorized representative.
The University will make every reasonable effort to ensure that confidentiality is maintained throughout the complaint and investigation process, to the extent consistent with the law, adequate investigation, and appropriate corrective action. This means that the University will share information only on a need-to-know basis.
