UC Berkeley recognizes that people with disabilities may require assistance in the event of an emergency. We therefore ask that all faculty, staff, and students with disabilities who believe that they may need assistance during an emergency evacuation complete this online confidential Self-Identification Questionnaire
The results will be accessible to the Office of Disability Access & Compliance (DAC) and shared with campus partners only to that extent required for facilitating assistance in an emergency. DAC can also help interested survey respondents develop a personal emergency plan that includes specific evacuation procedures, sheltering procedures, and means of communication in the event of an emergency.
